Waiver and Release of Liability

I, the undersigned, hereby acknowledge and agree to the following terms and conditions in consideration for being allowed to participate in dance classes, workshops, or events organized by [Hoyston Dance Center:

· Assumption of Risk

· I am aware that dance activities involve inherent risks, including but not limited to sprains, strains, fractures, and other injuries. I voluntarily assume all risks associated with participating in dance classes at the Studio.

· Physical Health:

· I certify that I am physically fit and have no medical conditions that would prevent my full participation in dance activities. I agree to inform the Studio of any health concerns or conditions that may affect my ability to participate safely.

· Release of Liability:

· I hereby release and discharge the Studio, its instructors, staff, and affiliates from any liability, claims, demands, actions, or causes of action arising out of my participation in dance activities at the Studio.

· Photography and Recording:

· I grant the Studio permission to use photographs and recordings of me taken during classes or events for promotional and marketing purposes, without compensation.

· Code of Conduct:

· I agree to abide by the Studio's code of conduct and follow the instructions and guidelines provided by the instructors for a safe and respectful learning environment.

· Emergency Medical Treatment:

· In the event of an injury or medical emergency, I authorize the Studio to seek and obtain medical treatment on my behalf.

· COVID-19 Considerations:

· Given the ongoing pandemic, you might want to include a section addressing COVID-19 risks and safety measures. Participants could acknowledge their understanding of the additional risks and agree to comply with any health and safety guidelines implemented by your dance studio.

· Code of Conduct and Studio Rules: I will follow all studio codes of conduct to perform better and create a healthy environment for my recreation.

At HDC, we are committed to creating a welcoming and respectful dance community. To ensure a positive experience for all participants, we ask that you adhere to the following Code of Conduct:

1. Respect for Others

Respect Instructors: Follow the instructions and guidance provided by our instructors. Treat them with courtesy and appreciation for their expertise.
Respect Peers: Be considerate and supportive of your fellow dancers. Encourage a culture of teamwork and camaraderie.

2. Punctuality and Preparedness

Arrive on Time: Be punctual for all classes, rehearsals, and events. Arriving on time shows respect for your fellow dancers and instructors.
Come Prepared: Bring the necessary dance attire, footwear, and any required equipment. Be mentally and physically prepared to engage in the activities.

3. Attire and Personal Hygiene

Appropriate Attire: Wear appropriate dance attire as specified for each class. This ensures a safe and comfortable environment for everyone.
Personal Hygiene: Maintain good personal hygiene. Use deodorant, and be mindful of perfumes or scents to create a comfortable space for all participants.

4. Studio Facilities

Care for Equipment: Treat studio equipment and facilities with respect. Report any damage or issues to the studio staff promptly.
Cleanliness: Help maintain a clean and organized studio space. Dispose of trash properly and keep personal belongings in designated areas.

5. Safety First

Follow Safety Guidelines: Adhere to all safety guidelines provided by instructors. Report any safety concerns to the staff immediately.
Warm-Up and Cool Down: Participate in warm-up and cool-down activities to prevent injuries and promote overall well-being.

6. Positive Attitude

Encourage Others: Celebrate the achievements of your fellow dancers. Create a positive and uplifting atmosphere within the studio.
Constructive Feedback: Provide feedback in a constructive and respectful manner. Embrace opportunities for improvement.

7. Communication

Open Communication: If you have concerns or questions, communicate openly with instructors and staff. We value your feedback and are here to assist you.
Studio Announcements: Stay informed by regularly checking studio announcements and updates.

By participating in activities at [Houston Dance Center, you agree to abide by this Code of Conduct. We believe that a supportive and respectful community enhances the dance experience for everyone.
